Kerala patients choose a clinic the way they choose most things now: they search Google Maps, ask in the family WhatsApp group and watch a doctor explain something on Instagram. Doctors, however, work under advertising rules that a textile shop never has to think about, which is why generic "grow your business" tactics go wrong in healthcare.
This playbook is the approach we take when planning marketing for clinics and hospitals: what the rules allow, how to set up Google so the right patients find you, which ads work without crossing the line, and how to measure bookings without exposing patient data.
What rules limit how clinics can market in Kerala?#
Four sets of rules apply: medical ethics rules, Kerala's clinical establishment law, other central laws on health advertising, and the ad platforms' own policies.
Medical ethics. In August 2023 the National Medical Commission put its new professional conduct regulations on hold. The Indian Medical Council (Professional Conduct, Etiquette and Ethics) Regulations, 2002 (opens in a new tab) therefore remain the operative code for doctors of modern medicine. Three parts matter most for marketing:
- Soliciting patients, directly or indirectly, is unethical for a doctor, a group of doctors or an institution. Doctors may not advertise themselves in a way that draws attention to their skill, qualifications or achievements, or that amounts to self-aggrandisement. Nor may they boast of cases, operations or cures.
- A doctor may make a formal announcement about starting a practice, changing its type or address, being temporarily absent, resuming practice, or publicly declaring charges.
- Patient photographs and case reports may not be published without the patient's permission when the patient can be identified.
Dentists and AYUSH practitioners answer to their own councils' codes, which set similar limits.
Kerala's clinical establishment law. Section 39 of the Kerala Clinical Establishments (Registration and Regulation) Act, 2018 requires clinical establishments to display the fee and package rates for each service in Malayalam and English, and bars charging more than the displayed rate. In November 2025 the Kerala High Court upheld the Act, in a case brought by hospital and doctors' associations, and directed establishments to publish service and rate information at reception and on their websites (opens in a new tab). Orders like this can be refined on appeal, so check the current position with your association; a clear rate page is good practice either way.
Other laws. The Drugs and Magic Remedies (Objectionable Advertisements) Act, 1954 bars adverts for remedies claiming to treat or cure the conditions on its schedule, which include diabetes and cancer. The PC-PNDT Act prohibits any advertisement about pre-natal sex determination.
Platform policies. Under Google's Health in personalized advertising (opens in a new tab) policy, ads for health conditions and treatments cannot use your own remarketing lists or Customer Match. Google's predefined audiences, location and search targeting still work. Meta's personal attributes policy (opens in a new tab) bans ad copy that asserts or implies a health condition. "Suffering from knee pain?" will be rejected, while "Knee physiotherapy in Kozhikode, book a consultation" is fine.
This is a practical summary, not legal advice. Check anything borderline with your council's code or a lawyer.
Where do Kerala patients look before booking?#
- Google Search and Maps. Searches name the need and the place: "dermatologist Kozhikode", "dental clinic near me". The map pack, your reviews and your listed hours decide most of these clicks.
- WhatsApp recommendations. A relative forwards your location pin, and the patient checks Google anyway, so your listing has to back up the recommendation.
- Instagram and YouTube. Doctors who explain conditions clearly in Malayalam earn trust long before a patient needs them. This is where ethics rules need the most care.
Keep your details on directories such as Practo identical to Google.
How should a clinic set up its Google Business Profile?#
Google Business Profile is usually the highest-return marketing a Kerala clinic can do. Google's guidelines for representing your business (opens in a new tab) allow one profile for the clinic, plus separate profiles for public-facing practitioners such as doctors and dentists who can be contacted at that address during stated hours.
- Name: use your real, signboard name. "Dr. Anjali's Skin Clinic" is fine; "Best Skin Clinic Calicut Laser Hair Removal" breaks Google's rules and your council's.
- Categories: choose the most specific primary category, such as Dental clinic, Dermatologist or Physiotherapist, and add a few accurate secondary ones. One practitioner should not have several profiles for different specialities.
- Hours: list real OP timings, and set special hours for Onam, Vishu, Eid and Christmas. Wrong hours lead to a patient arriving at a closed door and leaving a one-star review.
- Booking link: point it at your own booking page or WhatsApp, not a third-party directory that also lists your competitors.
- Photos: show the exterior and signage (Google expects permanent signage at your address), the reception, treatment rooms and equipment. Leave patients out.

How should a clinic handle Google reviews?#
Ask every patient, not just the happy ones, with a short link sent by WhatsApp after the visit. Never offer a discount or free consultation for a review; Google removes incentivised reviews.
When you reply, protect confidentiality. A reply that confirms someone was treated for a particular condition discloses patient information. Thank the reviewer, address the service issue (waiting time, billing) and invite them to call the clinic manager.
What should a clinic website include?#
A clinic website has two jobs: convert the patient who has just found you on Google, and rank for treatment searches. It needs:
- One page per service or treatment. Explain who it is for, what happens at the appointment, which doctor sees these patients, preparation needed and the fee or package rate. These pages rank for searches like "root canal treatment cost Calicut", and they meet the rate-display expectation.
- Doctor profiles. Give recognised qualifications, registration numbers, languages spoken and OP days. The 2002 regulations require doctors to display their registration number and to list only recognised degrees after their names.
- Rates and patient information. Publish services and package rates in Malayalam and English, the grievance contact and the date of the last update.
- A booking route. Offer click-to-call, WhatsApp and a short booking form that works on a phone, and show clearly when the request will be confirmed.
- Technical basics. Fast loading on 4G, clinic and physician schema markup, and an address and phone number that match Google exactly.
Treatment pages grow bookings for years, which is why SEO for treatment and location searches usually earns more than a clinic's ad budget over time.
Which ads work for clinics without crossing the line?#
Paid ads suit clinics that want bookings this month, a new branch or a new speciality. Search usually comes first, because it answers a need the patient has already named.
Google Search and call ads#
Bid on treatment and location terms ("dental implants Kozhikode", "ENT doctor near me"), and send each ad group to the matching treatment page. Add call assets and location assets, and track phone calls as conversions, because many patients call rather than fill in a form. Build a negative keyword list early. Terms such as "jobs", "course", "salary", "free", "MBBS" and "home remedy" drain clinic budgets quickly.
Meta ads for launches, camps and awareness#
Meta suits a new speciality, a health camp or a new branch. Keep the copy factual (service, place, date, how to book), and skip before-and-after images, which Meta's health policies restrict and ethics rules make risky. Lead forms should ask for a preferred date and branch, nothing clinical.
If you are weighing the two platforms, our comparison of Google Ads and Meta ads for Kerala businesses shows how to split a budget by the job each platform does.
What can doctors post on Instagram and YouTube?#
Educational content is the safest and strongest content a doctor can publish. Before posting, ask: does this help a patient understand something, or does it ask them to choose me? Explaining when a cough needs an X-ray is education; "Come to the best chest clinic in Malabar" is solicitation. Formats that work:
- Myth-and-fact reels in Malayalam, 45 to 60 seconds long, with the doctor on camera and captions for silent viewing.
- Seasonal explainers. Fever warning signs in the monsoon, heat illness in April and May, managing diabetes while fasting in Ramadan, and health checks for NRI families home for the Gulf summer holidays.
- "What to expect" videos that walk through a procedure room, an MRI or a first physiotherapy session, with no patients shown.
- Answers to common questions about fees, insurance and preparation.
Keep patient faces out unless you have written consent, and never frame a story as proof of a cure.
How do you measure clinic marketing without exposing patient data?#
Measure bookings, not diagnoses. Record the source of each appointment request (Google Ads, Maps, Instagram, referral) and whether the patient attended. Keep clinical details in your medical records system, away from marketing tools.
Two safeguards. Do not fire ad pixels with event names or URLs that reveal a condition: a "fertility-consult-submitted" event is health data. And collect clear consent on booking forms, since India's Digital Personal Data Protection Rules phase in core consent and security duties by May 2027.
What about patients in the Gulf?#
Many Kerala clinics see NRI families during the Gulf summer holidays in July and August, and some serve patients who travel from the GCC for treatment. Reaching Malayalis in the Gulf with Meta ads is possible, but advertising health services inside the UAE needs a health advertisement licence from the relevant authority (MOHAP, DHA in Dubai or DoH in Abu Dhabi). Someone also has to answer WhatsApp at Gulf hours.
Frequently asked questions#
Can doctors in Kerala advertise on Instagram?#
Doctors can share educational content and formal announcements such as a new clinic, timings or fees. The 2002 regulations prohibit soliciting patients or promoting a doctor's own skill, achievements or cures.
Can a clinic run Google Ads in India?#
Yes. Clinics and hospitals can run search, call and Maps ads for their services. The main restrictions are Google's healthcare policies on prescription drugs and unproven treatments, and its personalised advertising policy, which blocks remarketing and customer lists for treatment ads.
Can we use patient testimonials?#
Only with the patient's written consent, and even then avoid identifiable photos and anything that reads as a cure claim. Google reviews, written in patients' own words, are the safer route.
How much should a small clinic spend on Google Ads?#
It depends on your specialities and area. As a planning range for 2026, a single-location clinic testing search ads might budget ₹20,000 to ₹50,000 a month in ad spend, plus 18% GST. Treat that as a starting assumption rather than a benchmark, and adjust once you know your cost per booked appointment.
Is it allowed to offer a discount for a Google review?#
No. Google's policies prohibit offering discounts, free services or payment for reviews, and such reviews can be removed. Ask every patient for honest feedback, with no incentive.
Grow bookings without risking your registration#
Clinics that grow steadily in Kerala do the unglamorous things well: accurate Google profiles, honest reviews, clear treatment and rate pages, and search ads that answer real questions.
